Physical therapy

Software for School-Based Physical Therapy Companies

Prism K12 is built for practices whose physical therapists and PTAs serve students under IEPs across several schools and districts: schedule the year, write the notes, sign the plans of care, and show every district that the minutes were delivered.

Itinerant by default

Therapists cover several schools and districts. Schedules, caseloads and rosters span all of them.

PTs and PTAs together

Each clinician sees only their own caseload, and every plan of care states how the assistant is supervised.

The school year, not a referral

Recurring schedules are generated across the school year, and closures are recorded rather than counted as misses.

Answerable to the district

Delivered minutes against prescribed minutes, IEP dates and progress reports, ready before anyone asks.

Plans of care

Plans of care that start from PT wording, not a blank page

A new plan starts from the student’s active goals and the standard physical therapy wording your admin sets once: planned interventions, PTA delegation and supervision, the re-evaluation plan. The therapist fills in the diagnosis, prognosis and clinical rationale, and signs.

  • Interventions pre-filled: balance, gait, strengthening, postural control, range of motion and functional mobility
  • Diagnosis with its ICD-10 code, credentials and license number on every plan
  • Every prescribed service with no signed plan, or one expiring within two weeks, in one list
The plans of care list in Prism, flagging each student whose prescribed service has no signed plan

Daily notes

SOAP notes for the whole day in one sitting

Notes to write lists every session that still owes a SOAP note. Open one, fill in the subjective, objective, assessment and plan, and Save and next takes you straight into the next student.

  • A note counts as done only once all four sections are filled in
  • Your draft is saved as you type
  • Bulk export of SOAP notes for records requests

IEP service minutes

Know whether every student got the minutes their IEP prescribes

Each prescribed service line records the amount and whether it is delivered on an A grid (consultative), B grid (push-in) or C grid (pull-out). Prism compares what was delivered against what was prescribed, and school closures do not count against you.

  • Prescribed service lines on A, B and C grids
  • Alerts raised before an IEP expires
  • Every caseload across every school and district in one table

Progress reports

Progress on every IEP goal, already charted

Every data point lands on the goal it belongs to, so when a reporting period closes the chart is already drawn against the baseline and the target, with each session’s note beneath.

  • Built from the session data you already collected
  • Downloadable as PDF
A progress report in Prism charting one goal's session-by-session ratings on a 1 to 5 scale, with dashed lines marking the baseline and the target, and each session's SOAP note beneath

In the gym or the hallway

Take goal data mid-session on a phone

School PT rarely happens at a desk. Open the session on a phone, mark attendance and tap once per trial for each goal. The numbers are waiting when you write the note.

  • Attendance on every session, including no-school closures
  • A running count for each goal, saved against the session

Questions from physical therapy practices

Is Prism K12 built for school-based physical therapy?
Yes. Prism is built for companies that employ physical therapists and PTAs and contract with school districts: therapists who travel between schools, work from IEPs rather than prescriptions, and report progress on the school calendar.
Does Prism handle plans of care for physical therapy?
Yes. A plan of care starts from the student’s active goals and from standard physical therapy wording an admin sets once, including planned interventions such as balance, gait and strengthening work, PTA delegation and supervision, and the re-evaluation plan. Prism lists every prescribed service that has no signed plan, or one about to expire.
Can PTAs use Prism?
Yes. Every clinician on the team signs in with their own account and sees only the students assigned to them. Credentials and license numbers are recorded on each plan of care, and the plan states how the assistant is supervised.
How does Prism track IEP service minutes for PT?
Each student’s prescribed service lines record the service, the amount, and whether it is delivered on an A grid (consultative), B grid (push-in) or C grid (pull-out). Prism compares the sessions actually delivered against that prescription and flags shortfalls, without counting school closures against you.
Does Prism support occupational therapy or speech therapy?
Not yet. Occupational therapy and speech-language pathology support are coming soon. Prism is available today for physical therapy practices.
